Todos necesitan llenar estos formularios antes o durante registro escolar. Puede imprimirlos y traerlos para ahorrar tiempo. Si cambio su direccion durante el verano, por favor traiga 3 pruevas de la nueva direccion para actualizarla en el sistema. Para solicitar reembolso or para pagarle a un proveedor por un gasto que está en el presupuesto aprobado del PTSA, use este formulario de reembolso. Tendrá que adjuntar recibo(s) y/o factura con el formulario.

Everyone needs to fill out these forms either before or during school registration. You can print them and bring them to save time. If you’ve changed your address during the summer please bring 3 forms of proof of address to update the system. Also, if your child received new vaccinations please bring documentation.

Reimbursement and Check Request Form
To request reimbursement or to pay a vendor for an expense that's in the approved PTSA budget, please submit a reimbursement and check request form. You must submit appropriate receipt(s) and/ or invoices with the form.
